Functional Medicine Membership

This membership is perfect for individuals seeking a personalized approach to their health through functional medicine practices. It includes:

  • $399 Initial Consultation: Comprehensive assessment of your health and wellness goals.
  • $150 Patient Deposit: Applied towards the initial consultation.
  • Labs + Review Included: A lab review will be scheduled one week after blood work.
  • 6-Month Program: Designed to address your health concerns with tailored strategies.
  • $50 Monthly Fee: Ensures ongoing support and access to exclusive benefits.
  • $100 Telehealth Visits: Convenient virtual check-ins.
  • $150 Office Visits: In-person appointments as needed.
  • $150 No-Show/24-Hour Cancellation Fee: For missed or late-canceled appointments.

Membership benefits include a lab review after initial blood work and virtual check-ins every 12 weeks. Additional visits outside of these are available and charged per visit. Patients are responsible for notifying the clinic when additional medication is required.

Premier Platinum Membership

  • Monthly Price: $249 USD

    How do I schedule my virtual quarterly health check-ins?

    Can I cancel my membership?

    Yes, you can cancel your membership at any time. However, if you choose to cancel during your commitment period, you will be required to pay the difference for any discounted services received during your enrollment. We kindly ask for at least 48 hours' notice before the end of your commitment period if you wish to cancel.

    Are lab fees included in the Functional Medicine Membership?

    Yes, the membership includes one lab draw and review, providing comprehensive care and ensuring medical clearance before prescribing medications, peptides, antioxidants, and more. Labs must be updated every 12 months. Additional lab draws outside the initial one included with the membership will incur a $150 fee.

    What if I miss an appointment? Is there a cancellation policy?

    Yes, a non-refundable $150 fee applies for no-shows or cancellations made less than 24 hours in advance. We recommend notifying us promptly to avoid any charges.

    Can I combine membership perks with other promotions?

    Membership perks cannot be combined with other Fulcrum promotional offers unless explicitly stated.

    How do I schedule my virtual quarterly health check-ins?

    We will assist you in scheduling your quarterly virtual check-ins during your initial lab review. These are set every 12 weeks following the review. Additional follow-ups can be scheduled upon request and are subject to a fee of $100 for telehealth or $150 for in-office visits.
